Form C is a disclosure document filed with the SEC pursuant to Section 4(a)(6) of the Securities Act of 1933 and Rule 201 of Regulation Crowdfunding. It includes: (a) information about the issuer, its officers and directors, and beneficial owners of 20% or more of the company's securities; (b) a description of the business; (c) the terms of the offering; (d) a description of how the proceeds will be used; (e) related-party transactions; (f) risk factors; and (g) financial statements.
